Aware of possible risks and complications. The Shape You Want is a scheduling facilitator and not a healthcare provider. I am NOT relying on any statements made or implied by staff / employees of The Shape You Want, as I understand that they are not medical doctors. The surgeon performing my procedure is the only qualified person to medically treat me or give me medical advice. My decision to have this procedure performed is based on my own research and may be based in part on the advice received from my personal, licensed medical physician.
In addition, customers are required to know:
- Patients (clients) are required to be satisfied with the more common risks and complications of this surgical procedure and associated treatment(s) including but not limited to: bleeding, infection, anesthesia risks and death.
- Patients (clients) are required to be satisfied with the understanding of all risks and complications of this surgical procedure and associated treatment(s) including but not limited to: gastric bypass surgery, mini gastric bypass surgery, gastric sleeve surgery, gastric balloon, and any revision surgery.
- Patients (clients) are required to be satisfied with the understanding of the possible consequences, outcomes or risks if no treatment is rendered.
- Patients (clients) are required to understand that there are risks involved in any surgical procedure and associated treatment(s), and it is not possible to guarantee or give assurance of a successful result.
- Patients (clients) are required to be aware that they are fully responsible if they take it upon themselves to leave earlier than MGBG treatment requires; the hospital facility, nor the physician(s) are in any way responsible.
Understand The Shape You Want is NOT a healthcare provider. I understand that The Shape You Want, a division of MGBG, is an information and facilitating service and thus does not have any medical licences. I understand that this does not indicate a physician-patient relationship. I further understand that the staff, employees, coordinators, liaisons and anyone else from The Shape You Want are not medical doctors, and the information they provide should be gone over with my personal doctor.
Understand the Services MGBG provides. I am satisfied with my understanding regarding the nature of my surgery and the associated treatments; any questions about these treatments and procedures have been answered.
Recommend Flight Insurance on Travel. The Shape You Want highly recommends the purchase of Flight Insurance to cover trip cancellation and interruption. You can obtain this insurance when you purchase your airline tickets.’
